Cheshire Care helps individuals access their community


Outreach services are an important part of an individuals day to day routines and provides support to those who normally may not have access to these opportunities and activities.  This is a mobile service based in the individuals locale.


Outreach/domiciliary care is a a large part of this service where a carer visits the home of the individual to help and assist with daily activities that may be a struggle.  Each individuals needs are assessed, the time and frequency of visits and then the most appropriate support worker is assigned following the assessment.

We currently have facilities and resources for 10 individual clients and will assist with daily household tasks, medication administration, personal care and community access.   The overall aim being to assist the individual gain a degree of independent living.
